Whether antidepressants can trigger suicidal thoughts in some patients remains one of the most controversial questions in psychopharmacology. Clinical trials offer only part of the answer because they rarely include people with active suicidal ideation.

To address this gap, a study published in Pharmaceuticals analyzed adverse event reports from EudraVigilance, the European Medicines Agency's pharmacovigilance database, covering six widely prescribed antidepressants between 2001 and 2024.

Pharmacovigilance data cannot establish causation. A reported suicide does not mean the drug caused it, only that a possible link was suspected and reported. Even so, these reports can reveal important safety signals.

The weight of psychiatric adverse events

The study's broadest finding concerns the share of psychiatric reactions within the total pool of reported adverse events. Across the six drugs, that share ranged from 33.9% to 38.2%. Roughly one in three reported that adverse events involved psychiatric symptoms.

Two drugs stood out for different reasons. Venlafaxine led in absolute numbers, with 13,134 psychiatric reports, likely reflecting both its volume of use and its pharmacological profile as an SNRI. Duloxetine, meanwhile, had the highest relative share, 38.2% of all its reports.

Completed suicide, attempt, and ideation: Why the categories matter

The study analyzed five separate outcomes: completed suicide, suicidal behavior, suicidal ideation, suicide attempt, and suspected suicide. Looking at them individually revealed patterns that would have been missed if they had been grouped into a single category.
