A recently published report disclosed that a total of four serious patient-safety incidents occurred at the Norfolk and Norwich University Hospital (N&N) in 2025. 

Three of the incidents happened in September and involved wrong-site surgery, while another saw a foreign object left inside a patient following an operation in May. 

Known as “never events”, the errors are nationally defined as serious patient-safety incidents considered largely preventable when relevant safety guidance and recommendations are followed.
