A cyclist, a mother of two and Welsh triathlete described as a ‘fierce competitor,’ was killed when a white van driver misjudged an overtake on a dual carriageway.

The case is the latest to be featured on series five of Crash Detectives, a BBC show focused on Gwent Police.
